I just tried this not only with the slate tile, but various other rugs all to no avail.

I had the same problem. I got everything filled out and clicked "send request" only to have "YOUR ACCOUNT VERIFICATION INFORMATION DOES NOT MATCH OUR RECORDS" pop up. How can it not match? I checked and everything I entered is correct.

I might try calling them tomorrow if I think about it. If nothing else to find why my information doesn't match their records. I can't help but agree with the person who said that maybe now they know it's an issue they'll correct it system wide.
