Actually, you can get it by buying medicine from Dr. Quack. That's how I got mine __________________

The only way to get this badge is to have a sick pet that you bring to see Dr. Quack. You can't get it from playing the Dr. Quack's assistant.
Your pet has to have that green nose thing happening and can't go to work because Ms. Von Meow will tell you that your pet should go see the doctor before he comes back to work. | | | |

To follow up with what DixieCup said, you can't take a sick pet to work, or to school or even on vacation. They all tell you to take them to see Dr.Quack first. Quite cute actually...not the having a sick pet aspect, but that it's recognized by the other platforms. | | | |
