I finally got a trophy after playing at least 150 games, filling the report card at least once, sometimes twice, and occasionally 3 times. I don't know when I got it, since I didn't get the message saying I had won it. I just found it in my dock one morning.

Others have written good suggestions to help you win the trophy. The most useful (for me) was that you never get two of the same sticker at the same time. So if you are trying to get all three stickers, don't use any (if possible) till you have all three on the screen. For example, if you have an apple and a clock, and are waiting for the eraser, but you then use the clock, the next sticker to come down may be another clock, which you don't need. If you have two stickers on the screen, the next sticker to come down will be the one you need. (If possible, try to use all three in the same sum, and you will get a nice point bonus.)

On this game it also really helps to have a slow computer. Many people with faster ones report that they can only fill the reportcard once per game. I've got an old one and can do it three times (if I'm in the mood for some mental exercise).

Wow, I didn't realize computer speed makes a difference. I have a super fast computer and can only fill the card once per game. I have won one trophy, I have no idea how and have not been able to win again.

I won a trophy in January and not once since then.
